For your convenience, High Point Bank offers several forms that can be downloaded, printed and mailed or delivered to any of our locations. Because each of these forms requires an original signature, they cannot be emailed.

How to use this form:

  1. Read and print our Online and Bill Pay Users Guides.
  2. Click on highlighted link above.
  3. Complete all appropriate information.
  4. Print, sign, and date the form.
  5. Mail or deliver the form to any High Point Bank office.
  6. Keep the Online Services Agreement for your records.

Application for a High Point Bank CheckCard or ATM Card

Apply for our High Point Bank Check Card or ATM Card

A great way to receive cash and make purchases with your High Point Bank Checking Account!

Note: Our ATMs are located in the greater High Point/Jamestown community. If you use ATMs that are not owned by High Point Bank and Trust Company, we charge a $1.00 fee. The other bank may also impose a fee.

How to use this form:

  1. Click on highlighted link above.
  2. Read and Print the BankCard agreement.
  3. Click I accept or I do not accept at the bottom of the page.
  4. When file download box appears - select "run this program from current its location" then click "ok".
  5. After download is complete - a security warning box will appear - click "yes".
  6. These do not have a security certificate because they are forms and not actual programs.
  7. An Omniform information box will appear - click "ok".
  8. Complete all appropriate information online All areas highlighted in yellow can be completed online.
  9. Print, sign, and date the form.
  10. Mail or deliver the form to any High Point Bank office.

How to use this form:

  1. Read our online privacy agreement.
  2. Click on link above.
  3. Read the Online Banking agreement.
  4. Complete all appropriate information.
  5. Print, sign, and date the form.
  6. Keep the agreement and Mail or deliver the application to any High Point Bank office.

Notify us of your Change of Address

To change your address for all or some of your bank records, download the Change of Address Notification form. Once you have downloaded the form, please fill in all information, print the form, sign and date it, and either mail the form or deliver it to any High Point Bank location. Please note: this form requires an original signature, therefore it cannot be emailed.

Update your Financial Statement

This form allows you to periodically update information on your financial statement.
